First Time Applicants
Required Documents
First time applicants will need to fill out an Application for a passport, form #DS-11.
- Proof of United States Citizenship (only one of the following is required)
- Original State-issued Birth Certificate with raised seal
- Certified copy of STATE-issued birth certificate with colored seal (Note: CA & TX abstract of birth certificates are not accepted)
- Birth Records issued by a hospital are not accepted
- Original Naturalization Certificate
- You may be required to submit the following in addition to proof of citizenship:
- Original name change documents (marriage license, divorce decree, adoption papers
- Identification
- Current state-issued driver's license
- Current state-issued ID Card
- Current Military ID
- Photocopy of identification presented in #2
- Photocopy on "8 1\2 x 11" white paper showing the front and back of your form of identification used for #2.
- Passport Photos
- Two 2x2 identical, recent, passport photographs
- We provide this service for $15
- Fee Payments
- Application Fee: Check or Money Order payable to U.S. Department of State
- Execution Fee: Check, Money Order, Debit or Credit Card to CSULB Passport (The execution fee is set by the U.S. Department of State)
Child/Youth Passports
- Children under 16 years of age must apply in person with both parents present (for consent and proof of identity).
- Youths 16 or 17 years old must apply in person with one parent (for proof of identity).

Tuesday & Thursday: 9:30 -1:00pm
